Our Upperline Plus Registered Nurses (RNs) are available 24/7/365, offering around-the-clock triage support and guiding patients through the critical days and weeks after a hospital stay.
Whether they’re fielding urgent late-night calls or ensuring a smooth recovery at home, our nurses are more than clinicians—they’re lifelines. Their goal is clear: to ensure that every patient has access to compassionate, knowledgeable care whenever they need it most.
Who Are the Upperline Plus RNs?
The Upperline Plus RN team plays a vital dual role in care continuity:
- Triage Nurses: Available day and night to answer health-related questions, assess urgent needs, and coordinate care alongside nurse practitioners.
- Transitions of Care (TCM) Nurses: Follow up with patients within 24 – 48 hours after hospital or ER discharge to ensure follow-up care is arranged and no details are overlooked.
Beyond clinical support, our nurses act as trusted educators and advocates—building relationships, ensuring continuity with care, and helping patients overcome barriers to recovery.
Always On Call: The Power of Nurse Triage
When a patient can’t reach their doctor, they don’t have to wait or worry—help is just a phone call away with our nurse triage line. From treating urinary tract infections and seasonal allergies to chronic condition management, and getting home labs and x-rays ordered, our triage nurses provide care that helps patients avoid unnecessary urgent care visits and receive the support they need from the comfort of home.
“I talked to a patient yesterday,” shares Upperline Plus RN Melanie Loggains, “who didn’t have much support. She had a UTI and was scheduled for surgery. We got her on antibiotics Saturday, and she was able to have her wrist surgery on Monday. She called afterward just to say thank you. She said she thinks of us as family.”
That kind of personal connection—someone kind, capable, and caring who picks up the phone—is what makes this team’s approach extraordinary.
Healing After Hospitalization: Transitions of Care
Leaving the hospital can be overwhelming: pages of instructions, new medications, follow-up appointments, and equipment needs. Our Transitions of Care RNs ease that burden.
“We call within 24–48 hours of discharge,” says Jamie Pursell, a TCM RN. “We make sure patients have their medications, equipment, and know when and where their next appointments are. We don’t let anything fall through the cracks.”
Patients continue to receive follow-up calls for 30 days post-discharge. Nurses coordinate with providers, assist with referrals and equipment orders, and—most importantly—empower patients to understand and manage their care.

Caring for the Whole Person: More Than Just Medical Support
Upperline Plus RNs go beyond medical charts. They listen closely to patients and respond to any unmet needs in areas such as food security, transportation, housing, and access to social services; the non-medical barriers that can impact healing and quality of life.
- Do they have enough food?
- Can they get to their next appointment?
- Are they safe at home?
- Do they need help applying for assistance programs?
“We listen for more than symptoms,” says Melanie. “If someone’s struggling with groceries or heat, we connect them with local resources—social workers, 2-1-1, the Office on Aging. Whatever it takes.”
This holistic, human-centered approach makes a lasting difference. It’s not just about care—it’s about dignity, safety, and quality of life.

“We all come from different places—ER, chronic care, family practice,” says triage RN Lennon Reed. “But we work so closely that when a patient calls, they don’t just get one of us—they get all of us.”
