This is the most secure and efficient way to ensure your personal health information is shared with you in a timely and confidential manner.

For Third-Party Requesters

(i.e. Attorneys, Insurance Companies, and Auditors)

To request medical records on behalf of a patient, please complete the medical records request form. 

This form is HIPAA-compliant and ensures the protection of patient privacy and enables us to fulfill your request in compliance with applicable laws and regulations.
