About Jeremy Leaf
Jeremy Leaf, DNP, FNP-BC, is a dedicated and compassionate Family Nurse Practitioner with a Doctor of Nursing Practice degree from Augusta University and over a decade of clinical experience. Currently practicing at Upperline Health in Augusta, Georgia, he delivers high-quality primary and acute care to a diverse patient population with a strong focus on evidence-based medicine and patient-centered care. His career began in the cardiovascular ICU and telemetry, where he developed advanced skills in cardiac monitoring, critical care, and team leadership before transitioning into advanced practice.
Jeremy’s broad clinical background spans primary care, geriatrics, corrections, wound care, and the Veterans Health Administration, with expertise in chronic disease management, interdisciplinary pain management, and clinical education. He has been recognized for his ability to foster strong provider-patient relationships, mentor peers, and contribute to innovative care models such as in-home assessments and telehealth. He is especially passionate about providing accessible, empathetic, and efficient care to underserved and complex patient populations.
When not in the clinic, Jeremy enjoys fishing, camping, and playing guitar. These pursuits reflect his grounded, authentic approach to life—one he brings into every patient encounter as well.
